If you are looking to refer someone for services, here is what you need to know.
Who Qualifies?
We serve individuals with intellectual and developmental disabilities enrolled in one of the following Indiana Medicaid waiver programs:
- ✓Family Support Waiver (FSW) covers Participant Assistance and Care (PAC), Respite Care, and Day Habilitation services.
- ✓Community Integration and Habilitation (CIH) Waiver covers Respite Care, Day Habilitation, and Residential Habilitation and Support services.

What to Have Ready
Having the following information available will help us move through the referral process smoothly:
- ✓Individual's full name and date of birth
- ✓Current waiver type (FSW or CIH)
- ✓Case manager name and contact information
- ✓Brief description of support needs and goals
- ✓Any current service providers (if applicable)
- ✓Preferred contact method for the family or guardian
