Support That Starts With the Individual

Dynamic Possibilities offers four core service programs funded through Indiana’s Family Support Waiver (FSW) and Community Integration and Habilitation (CIH) waiver. Every program is built around the person it serves. We work closely with individuals, families, and case managers to create plans that promote independence, community participation, and a meaningful daily life.

Assessment and Planning

We schedule an in-person or virtual meeting to understand the individual’s goals, strengths, and needs. Together, we build a person-centered plan.

Begin Services

Once the plan is approved, we match the individual with a dedicated caregiver and services begin. Ongoing communication ensures the plan continues to work.

Participant Assistance and Care (PAC)

Family Support Waiver (FSW) | All Ages

PAC services are designed to help individuals with intellectual and developmental disabilities live successfully in their own homes, participate in their communities, and avoid institutionalization. Our team provides hands-on assistance, prompting, reminders, and monitoring to support daily living activities, self-care, and mobility.

Every PAC plan is individualized. We meet each person where they are and build a support structure that reflects their preferences, routines, and long-term aspirations. The goal is always to strengthen independence while ensuring health and safety.

Respite Care (RSPO)

CIH and FSW Waivers | All Ages

Respite services provide short-term relief to the families and caregivers who are the everyday pillars of support for their loved ones. When primary caregivers need time to rest, attend to personal responsibilities, or simply recharge, Respite Care ensures that the individual continues to receive safe, comfortable, and familiar support.

Services can be provided in the individual’s home, in the respite caregiver’s home, in a camp setting, in a DDRS-approved Day Habilitation facility, or in an approved residential setting. We match individuals with respite caregivers they can feel comfortable and safe with.

Day Habilitation (DHI and DHGS)

CIH and FSW Waivers | All Ages

Day Habilitation services focus on building practical life skills and meaningful social connections. Activities support learning in the areas of self-care, sensory and motor development, socialization, daily living skills, communication, community living, and social skills. Every activity is designed to build relationships and natural supports.

Our Day Habilitation programs are offered in individual and small group settings, ensuring that each person receives the attention and engagement they deserve. Programs are specified in the individual’s PCISP and are designed to be both purposeful and enjoyable.

Residential Habilitation and Support (RH10/RH20)

CIH Waiver | Supplemental Staffing Available

Residential Habilitation services provide individually tailored supports that assist with the acquisition, retention, and improvement of skills related to living in the community. These supports include adaptive skill development, assistance with activities of daily living, community inclusion, transportation, adult educational supports, and social and leisure skill development.

Through our supplemental staffing model, we also partner with other providers to support individuals already receiving residential services. If a primary provider’s staff member exceeds their 40-hour week, they can transition to Dynamic Possibilities for supplemental hours, maintaining continuity of care and trusted relationships.

Services FAQ

The Family Support Waiver (FSW) provides services for individuals living with their families. The Community Integration and Habilitation (CIH) waiver supports individuals living more independently in the community. Each waiver covers different services. Contact us to determine which applies to your situation.

Yes. PAC, Respite, and Residential Habilitation services can all be provided in the individual’s own home. Day Habilitation is provided in community and approved facility settings.

Individuals must have an intellectual or developmental disability and be enrolled in an FSW or CIH waiver through Indiana. If you are unsure, reach out and we will help you determine eligibility and connect you with the appropriate resources.

Our supplemental staffing model allows caregivers who have reached their 40-hour limit at a primary provider to continue supporting the same individual through Dynamic Possibilities. This preserves the trusted relationship, reduces overtime costs, and ensures uninterrupted care.

At this time, we do not provide 24-hour residential staffing. We currently offer supplemental staffing for individuals receiving services in their own home or who already reside in a provider-owned home and need additional support hours.

After the referral and assessment process, services typically begin within one to two weeks. The exact timeline depends on individual needs, waiver authorization, and caregiver matching.
