Who We Serve
Day Habilitation is available to individuals enrolled in Indiana’s Community Integration and Habilitation (CIH) Waiver or Family Supports Waiver (FSW). These programs provide structured learning, skill-building, and socialization opportunities in both individual (DHI) and group (DHGS) settings.

Support with personal hygiene, grooming, dressing, and other daily routines that build independence and confidence.

How It Works
Starting Day Habilitation is a collaborative process. We work with individuals, families, and case managers to create the right fit.
Referral
Contact us or have the case manager reach out to discuss the individual’s interests, goals, and waiver enrollment.
Assessment
We meet with the individual and their team to understand what they want to learn, experience, and achieve through the program.
Program Begins
The individual joins a program tailored to their goals. We provide regular updates and adjust the plan as they grow.
